Guy was born on April 27th 1894 in the Cleveland Township of Le Seur County, Minnesota. HIs father, Sidney J., and his mother, Addie Ellingsworth (maiden name), were both born in the USA. Their residence was at Elysian, Le Seur County, Minnesota. At the time of his enlistment his residence was in Morristown, Rice County, Minnesota. However he listed his home address to be that of his parents. He also had a sister, Mae, that lived with their parents. Guy was single and had completed 9 years of education. He was employed an a Flour Packer at the Morristown Mill. He was buried on October 6, 1918 at the Cedar Hill Cemetary of Elysian.
Military Service Record:
Guy enlisted on May 31, 1917 at Faribault, Rice County, Minnesota as a Private and was issued the serial number 1,426,420. He was assigned to the Machine Gun Company of the Minnesota National Guard 2nd Infantry which became the 136th Infantry of the 34th Division of the Army. The locations of his service were initially at Faribault, Minnesota; then training at Camp Cody, New Mexico; and then preparation to embark for France at Camp Dix, New Jersey. On November 16, 1917 he was promoted to Private 1st Class. On February 1, 1918 he was promoted to Corporal. On June 1, 1918 he was promoted to Sergeant. On September 29, 1918 he died of bronchial pneumonia at the Base Hospital, Camp Dix, Trenton New Jersey.
